5-30-13 assisted living problems

OSHKOSH, Wis. (AP) — An assisted living company is giving up control of most of its properties after racking up $100,000 in state fines related to deaths and injuries of residents.  Stephen Levy, executive vice president and general counsel for Senior Lifestyle Corp., says his company plans to take control of 10 assisted-living homes operated by CRL Senior Living Communities.  Levy wouldn't name all the facilities his Chicago-based company plans to operate but confirmed they include Bella Vista and Bella Vista Manor in Oshkosh.  An Oshkosh Northwestern investigation recently reported violations at many of CRL's 14 assisted living homes in Wisconsin since 2011.  State records documented deaths, neglect, injuries, dementia patients getting lost, fights and medication mix-ups.
